- ItemBenchmarking total knee replacement constructs using noninferiority analysis: the New Zealand joint registry study(BioMed Central Ltd, 2021-08-23) Wyatt MC; Frampton CF; Whitehouse MR; Deere KC; Sayers A; Kieser DBACKGROUND: The aim of this study was to compare the relative performance of total knee replacement constructs and discern if there is variability in performance in currently commonly used prostheses in the New Zealand Joint Registry (NZJR) using a noninferiority analysis. METHODS: All patients who underwent a primary total knee replacement (TKR) registered in the NZJR between 1st January 1999 to June 2020 were identified. Using a noninferiority analysis, the performance of total knee replacement prostheses were compared with the best performing contemporary construct. Construct all-cause revision rate was estimated using the 1-Kaplan Meier survival function method to estimate net failure. The difference in all-cause revision rates between the contemporary benchmark and all other constructs was tested. RESULTS: In total 110 183 TKR were recorded and 25 constructs (102 717 procedures) had > 500 procedures at risk at 3 years post-primary of which 5 were inferior by at least 20 % relative risk of which, one was inferior by at least 100 % relative risk. 14 constructs were identified with > 500 procedures at risk at 10 years with 5 inferior by at least 20 %, of which 2 were inferior by > 100 % relative risk. CONCLUSIONS: We discerned that there is great variability in construct performance and at all time points, greater than 25 % of constructs are inferior to the best performing construct by at least 20 %. These results can help inform patients, clinicians and health care funders when considering TKR surgery.
- ItemHow do cemented short Exeter stems perform compared with standard-length Exeter stems? The experience of the New Zealand National Joint Registry(Elsevier Inc on behalf of on behalf of The American Association of Hip and Knee Surgeons, 2020-03-03) Wyatt MC; Poutawera V; Kieser DC; Frampton CMA; Hooper GJBackground: The standard Exeter (Stryker) cemented stem is 150 mm long with standard offsets ranging from 37.5 mm to 56 mm. Exeter short stems of 125 mm are also available in the offsets of 37.5 mm, 44 mm, and 50 mm. In addition, smaller (125 mm or shorter) Exeter cemented stems with offsets of 35.5 mm or less are available. The aim of this study was to examine the New Zealand Joint Registry (NZJR) comparing medium-term survival rates and functional outcomes of standard-length stems with Exeter short stems of various offsets in patients undergoing primary total hip replacement. Methods: Using the NZJR, we compared the results of 3 separate groups of patients with Exeter stems. Patients with standard 150 mm length Exeter stems (Standard) were compared with patients with Exeter 125 mm stems with regular 37.5 mm, 44 mm, and 50 mm offsets (Short 37+) and Exeter 125 mm stems with offsets of 35.5 mm and below (Short 37−). Demographic data, preoperative diagnosis, patient-reported outcome measures, and reasons for revision were compared between groups. Kaplan-Meier survival analysis and Cox multivariate regression analysis were used to examine implant survival and the influence of stem group on revision rates adjusting for gender, age, diagnosis, and surgical approach. Results: There were 43,427 Exeter cemented stems in the NZJR between January 1, 1999 and 31, May 2018; 41,629 Standard, 657 Short 37+, and 1501 Short 37−. In all 3 groups, the posterior surgical approach was preferred (Standard, 76.1%; Short 37+, 94.6%; Short 37−, 76.6%; P < .001). In the Short 37− group, 94.1% were female, while in the other 2 groups, there was an equal gender ratio (P < .001). The Short 37- group was also significantly younger than the other 2 groups with 41.6% younger than 65 years compared with Short 37+ (37.2%) and Standard groups (36.9%) (P < .01). There was no difference in American Society of Anesthesiologists grade between groups. Body mass index (BMI) was significantly higher in both the Short 37− and Short 37 + groups compared with the Standard group (Standard BMI, 28.71; SD 5.72; Short 37+ BMI, 29.69; SD, 6.67; Short 37− BMI, 29.09; SD 7.07; P < .001). The all-cause revision rate for standard stems was 0.55/100 component years (cy) (95% CI: 0.52 to 0.58). The Short 37− group had a higher rate of revision compared with the Standard group (hazard ratio 1.6; 95% CI: 1.3 to 1.98; P < .001), while the Short 37+ group had a hazard ratio of 0.84 (95% CI: 0.38 to 1.88; P = .674) compared with the Standard group. Cox regression analysis controlling for age, gender, diagnosis of OA, and surgical approach did not affect these findings. However, no clinically meaningful difference between Oxford hip scores was observed. Conclusions: There was a significant difference in revision rates for aseptic loosening with standard-length Exeter stems having a lower revision rate than short Exeter stems with offsets 35.5 mm or less. The Short 37+ groups, despite comprising relatively small numbers, performed similarly to the Standard stem group.
- ItemThe anterior femoral cortical window as an alternative to an extended trochanteric osteotomy in revision hip arthroplasty surgery: the evolution of the surgical technique and outcomes in 22 consecutive cases.(SAGE Publications, 2024-08-20) Morley D; Wyatt MC; van Dalen JBackground:The anterior femoral cortical window is an attractive alternative to the extended trochanteric osteotomy when removing femoral cement in revision hip arthroplasty. CT-based additive manufacturing technology has now permitted the creation of patient-specific instrumented (PSI) jigs to facilitate this. The jig simplifies creation of the window, potentiating medullary exposure through an optimally-sized window and therefore cement removal. Between 2006 and 2021 this technique was used in 22 cases at a regional hospital in New Zealand (mean age 74; range 44 to 89 years). 16 cases were for aseptic loosening and 6 for infection. We describe the technique and our case series. Bone incorporation for the cortical window was assessed in all cases using CT imaging. Oxford scores were obtained at a minimum of 6 months after revision surgery. Of the 6 septic cases 5 went onto successful stage-2 procedures, the other to a Girdlestone procedure. Results:The mean rectangular shaped window size was 8 × 1.5 cm and in each case, this provided adequate intramedullary access. On average at minimum 5 months post-surgery, 84% bone incorporation of the cortical window occurred on CT (40–100%). The functional outcome Oxford hip score was 37 (range 22–48) for 10 cases. There were 2 cases with femoral component subsidence which then stabilised. Conclusions:This technique description and retrospective case series has shown the effectiveness of removing a distal femoral cement mantle in revision hip arthroplasty using an anterior femoral cortical window, recently optimised using a PSI jig. This technique is a straightforward alternative to a trochanteric osteotomy. Reliable bony integration of the cortical window occurred and functional outcomes were comparable with the mean score for revision hip procedures reported in the New Zealand Joint Registry.
